Sports Physicals are valid for one school-year only (June to June). Please contact your primary care physical to schedule an appointment if you are unable to attend the annual Sports Physical clinic held in May/June at Identity Medical Group by Dr. Mike Tushla. Sports physicals may be available by appointment at select locations:
